Connie Earl, DO, is a board-certified, family medicine-trained physician enrolled in Maine-Dartmouth Family Medicine Residency's ONMM Residency Program. She sees and treats patients at Maine Dartmouth Collaborative Care Center in Augusta. Before her fellowship, Dr. Earl worked 13 years in Sonoma County, CA at a federally qualified health center (FQHC), providing full-spectrum family medicine to an underserved community. She also served as chief medical officer of innovation at West County Health Centers, medical director of the Forestville Wellness Center (a stand-alone clinic that provides integrative care and group visits within a FQHC) and director of obstetrics.
