National Diabetes Prevention Program

If you're an adult at risk for developing type 2 diabetes or have pre-diabetes, the National Diabetes Prevention Program can give you tools to stay healthy and prevent the disease.

The program is free and provides access to a lifestyle coach in a weekly one-hour group setting. The group meets for 16 weekly sessions, then every other week for the rest of the year.

Programs are offered in local fitness centers, community settings or in primary care offices.

This program’s focus is on gradual lifestyle changes, resulting in fat and calorie reduction and increased physical activity.

Peer Support for Chronic Disease

Our trained volunteers can support you in managing ongoing chronic health conditions such as heart disease, diabetes, MS, chronic pain, weight concerns, arthritis, scleraderma, family caregiver concerns and other health conditions.

Known as "peer navigators," they provide support over the phone or in person that includes:

  • Providing encouragement and a listening ear
  • Helping you find ways to manage your condition better and work toward your health goals
  • Helping you find needed health care services and community resources
  • Joining you for a walk, an exercise class or just to sit and have coffee together

Peer navigators are especially helpful because they share their experiences of managing their own conditions.
