Our Facility
Located in a quiet, wooded area only minutes away from the conveniences of Augusta, Gray Birch offers a beautiful, scenic living environment.
The facility is home to three distinct levels of care, allowing for easy transition through the long term care continuum while providing high-quality, relationship-based care services.
Skilled Nursing Care and Rehabilitation
Our experienced, caring team of nurses and therapists are committed to offering patients the excellent care they need to return home as soon as possible.
Our staff is dedicated to making every patient's stay comfortable and successful while helping them regain independence in a supportive, welcoming environment.
Long-Term Nursing Care
Individuals who need a longer stay and need extensive help with activities of daily life can choose to stay in our long-term care community. It is our goal to promote independence, choice and resident preferences in each resident’s life despite their medical condition or level of care.
Assisted Living - The Residence
For seniors who can function relatively independently but require a supportive environment, Gray Birch offers The Residence at Gray Birch, a residential care community.
Our focus is to be resident- and family-centered in our services at The Residence. We engage with residents and strive to create a pleasant, home-like casual living environment.
The Residence offers residents assistance with medications and activities of daily living. Dining, laundry, and activities are all services offered to residents.
Activities
Our dedicated activities professionals work to provide a variety of activities and special events for residents at Gray Birch. We offer:

- Shopping trips
- Various locations throughout Maine including the coast, fishing trips and trips to fairgrounds
- Live music several times each month

- Church services, including non-denominational services

Infection Control
Gray Birch remains committed to the health and safety of all residents and staff. We continue to follow CDC guidelines for prevention and response to illnesses like COVID-19 and influenza.
Our Team
The Gray Birch staff receives extensive training. Our training program is updated each year to ensure the team remains highly skilled to care for our residents' unique care needs. Our staff includes:
- Registered nurses onsite 24/7
- Certified nursing assistants (CNAs) and primary nurses with dedicated assignments
- Care managers experienced in helping families and residents
- Physical, occupational and speech therapists with special training in rehabilitation services
- Onsite medical providers skilled in acute, skilled and long-term care services

We readily accept volunteers at Gray Birch and welcome what they add to residents' lives and our program. Volunteers can help in many ways, including:
