Patient-Centered Medical Home

You are our partner in care. We listen and provide the care and education that our patients deserve. We treat not only illness, but focus on prevention and health education and the well-being of the family as a whole.

Our team of health care professionals and support staff value a healthy lifestyle, teamwork and patient- and family-centered care. We use the latest evidence-based (proven) knowledge and science to help you prevent illness and manage your health, with help from your support network.

What You Can Expect

As your medical home, here's what you can expect:

• A dedicated primary care provider (PCP) who will get to know and care for you and/or your family
• An expanded team of staff, professionals and other care providers will partner with your PCP in your care
• A higher level of communication. We receive and respond to all of your test results and health information. We will be the home base for all of your medical information. Please share all health information with your PCP and our office.
• Counseling, education and community services and support are offered on-site.
