Financial Assistance: Long-Term Care
Having a difficult time understanding the variety of long-term care options and how to pay for them? We can help.
Our financial counselors can provide you and your family/caregivers with a long-term care application and some general information on what it requires.
You can contact the Maine Department of Health & Human Services (DHHS) Long-Term Care office directly at 1-855-797-4357, #3, where their staff will guide you through the application process, answer your questions and help you complete the needed applications.
Our goal is to help you make the best choices for you and your family. We want everything to go smoothly for you when you leave the hospital. If you need a higher level of care once you leave, we will help with this.

To obtain a long-term care application, please call us at (207) 872-4680 or toll-free at 1-877-255-4680, option 3. We are happy to mail an application to you. If you would like to meet with a financial counselor and/or pick up the application in person, we recommend that you call ahead to be sure a counselor is available so you don't have to wait.
We are available Monday through Friday, 8 a.m. to 4:30 p.m., and are located at:
- Alfond Center for Health, 35 Medical Center Parkway, Augusta
- Thayer Center for Health, 149 North Street, Waterville
- Hathaway Creative Center, 10 Water Street, Waterville
